To join MeridianHealth, please call the Illinois Client Enrollment Services (ICES) at 877-912-8880. You will be asked to choose an in-network Primary Care Physician (PCP) with MeridianHealth. To see if your PCP participates with MeridianHealth, use our Find a Doctor tool or call MeridianHealth Member Services at 866-606-3700. 

If you need to select an in-network PCP, you can call Member Services or use our online Provider Directory. 

If you have questions about enrolling in Illinois public aid, please visit your local DHS office.
