How do I get insurance through MeridianChoice?

We’re glad you’re interested in a MeridianChoice plan! Our  online enrollment tool is an easy way for you to buy one of our plans directly through us. With the same great health benefits and coverage, including all mandated 10 essential health benefits and free preventive care services, enrolling with a MeridianChoice plan is quick and simple.

How can I pay?

We proudly offer several payment options for your convenience:

  • Online: Our member portal allows you to pay your premium online. Click here to login
  • Mail: Send a check or money order with the coupon attached to your billing statement to the address indicated. We do not accept cash payments.

Is my doctor covered?

All of our health plans have comprehensive provider networks, allowing our members to get the right care at the right time by the right provider. But it’s good to double check. Here are two ways to check if your doctor is covered:

1. Visit our online Find a Doctor tool to see if your doctor and/or hospital is in the MeridianChoice network
2. Call us toll-free at 855-537-9746 (TTY 711), Monday - Friday from 8 a.m. – 8 p.m.

Population Health Management Programs

Our Population Health Management Programs are offered to MeridianChoice members and designed to improve your overall health and quality of care. Please review the various programs below. For more information, or to find out how to get enrolled, please contact MeridianChoice at 855-537-9746.

Catching a Breath

The Catching a Breath Program provides member education on chronic obstructive pulmonary disease (COPD) and other chronic conditions such as diabetes, hypertension and asthma. This program aims to improve self-management techniques and identify risk factors through the use of educational materials and assessment completions. In addition, Meridian partners with American Therapy Centers to provide an eight-week in-home respiratory therapy program for applicable members.


Complex Case Management

Meridian's Complex Case Management Program assists members with all their healthcare needs by giving them tools to combat their chronic conditions and navigate their appointments and benefits.

Flu Outreach

The Flu Outreach Program is an educational program used to increase accessibility to the flu vaccination in order to add value towards preventative care and keeping members healthy.



The Opioids Program is a set of initiatives intended to combat issues surrounding opioids prescribed through multiple pharmacies, multiple providers and at a high dosage.

Risk for Kidney Failure

The Risk for Kidney Failure Program provides educational outreach intended to help provide resources to those who are at risk for developing end stage renal disease (ESRD).

Smoking Cessation

The Meridian Smoking Cessation Program offers education materials, a personal Smoking Cessation Coordinator and coaching calls to help members quit smoking.


The National Committee for Quality Assurance has awarded MeridianChoice an Accreditation status of Accredited for service and clinical quality that meet or exceed NCQA's rigorous requirements for consumer protection and quality improvement.
