Note: You must get a referral from your primary care provider (PCP) to see a specialist. You must also get prior authorization (PA) for most services. You can get a PA by having your doctor fax a Prior Authorization Form to 313-463-5254 or call 855-537-9746. Services rendered in the member's PCP office do not require prior authorization. Primary Care Provider (PCP) offices that have CLIA certification do not require an authorization for labs performed within the PCP office. PCP offices that have a PCP "ORDERING" a CLIA waived tests to be performed at a Hospital that participates in MeridianChoice, do not require an authorization.
