MeridianComplete’s (Medicare-Medicaid Plan) Provider and Pharmacy Directory provides a list of the entire network of providers and pharmacies that you have access to as a member of our plan. In most cases, you must use in-network providers and pharmacies while you are a member of our plan. Please review your Member Handbook, or visit the Out-of-Network (OON) Coverage page to review  MeridianComplete’s OON provider and pharmacy access rules.

You must go to your assigned Primary Care Provider (PCP). They will refer you to other providers you may need to see, such as a specialist. You can change your PCP at any time by calling Member Services at 1-855-323-4578 (TTY: 711), Monday - Sunday from 8 a.m. – 8 p.m.

 

Member Services can help you find a PCP, specialist or other healthcare provider in your area as well as find a provider who speaks your preferred language.

You can view or download the most up-to-date version of the directory below. If you would like a Provider and Pharmacy Directory mailed to you, you can call Member Services, or email meridianmedicare@mhplan.com.

The List of Covered Drugs and/or pharmacy and provider networks may change throughout the year. We will send you a notice before we make a change that affects you.

 

For certain kinds of drugs, you can use the plan’s network mail-order services. We also have an optional automatic mail-order delivery program under which we will automatically fill all new prescriptions your health care provider sends to us, as well as refills for prescriptions that have already been filled but are running out. Usually a mail-order pharmacy order will get to you in no more than 5 days. If you experience any problems receiving your mail order prescription, call Member Services at
