DISEASE MANAGEMENT


MeridianHealth has special Disease Management programs to help our members with asthma, diabetes, CVD (heart problems) and COPD (lung problems) stay healthy. 

 

MeridianHealth's Disease Management program give you access to support from nurses and healthcare staff as well as educational materials and newsletters to help you learn about your disease.

 

You can join our Disease Management program in one of three ways: 

 

  1. If you have asthma, diabetes, CVD or COPD, you may be signed up for the program automatically by MeridianHealth.
  2. You can sign up for the MeridianHealth Disease Management program yourself by calling 866-606-3700
  3. . You can stop participating at any time.
  4. Your doctor can sign you up for the program.

MeridianHealth's respiratory disease management program covers asthma and chronic obstructive pulmonary disease (COPD) conditions. The goal of the program is to help you manage your asthma or COPD symptoms and keep your disease under control.

MeridianHealth's Diabetes Disease Management program is designed to support the treatment plan your doctor has developed for you and to assist you with controlling your disease.

MeridianHealth's Heart Disease Management program can help you manage heart conditions and prevent long term complications. Our Heart Disease Management program can assist members with heart failure or coronary vascular disease (CVD).

Members enrolled in our weight management program will have access to a Nutrition Care Coordinator. These coordinators help members stay on track with their weight loss goals and refer them to support services such as nutritional counseling and the Weight Watchers Program (which are both covered benefits for qualifying members).

 

Members also have access to our on staff Registered Dietician and Nutrition Specialists. To take advantage of this service, please call the Disease Management department at
